Manx Care is reminding people to only use the emergency department at Noble's Hospital if they absolutely need to.
The health body says the facility is operating under 'extreme pressure' once again.
However, it assures a 'clearly defined plan has been activated to manage this and ensure patient safety is maintained'.
The advice has been issued a number of times in recent weeks, with admissions to A&E for respiratory illnesses at the highest level in five years.
