Review found vulnerable teen almost died following incident
More training is needed after a vulnerable young person in the Island's care system almost died from a drug overdose.
That's from the chair of the Safeguarding Board following an independent review.
A number of recommendations have been made after the report found moments to prevent the incident had been missed.
